At Blue Sky Assisted Living in Arizona we do our best not only to provide the best care, but also educate on the financial resources available. As a veteran, or surviving spouse of a veteran you have great financial resources available to you. Usually between $1100-1700 a month if assisted living is needed. With many assisted living facilities that wouldn’t even cover half, but at Blue Sky that will cover the majority of the expense.
The requirements include serving at least 90 days, 1 day during war time, and a need for assisted living care. The process can take several months and a lot of paperwork, but well worth the great benefits. Just so you know what to get together below is a list of paperwork that will be needed:
- Discharge separation papers
- Copy of marriage certificate
- Death certificates (surviving spouse only)
- Net worth information including banks accounts, trust, stocks, bonds, annuities, CD’s and so forth
- Copy of current social security letter
- Proof of medical expenses that are not covered
- Physician statement including diagnosis, prognosis ect.
- List of doctors visited in the last year.
Once you the this information together you will need to complete the following forms
VA Form 21-527EZ (for veterans with instructions included)
VA Form 21-534EZ (for spouses with instructions included)
When you mail these forms in it is best to get a receipt so that you can get a confirmation, and VA can’t say they didn’t receive the information. Also remember to follow up frequently to ensure things are progressing. Don’t worry about bothering anyone, just keep in constant communication.
